Welcome to the Christian Family Solutions Screening Program. Christian Family Solutions is collaborating with your school to provide a mental health screening for faculty and staff. This is a voluntary screening used to help identify indicators of well-being as well areas of risk of experiencing social, emotional, and/or behavioral difficulties. Once this online risk assessment has been completed, an appointment will be scheduled on campus at school or through a secure video connection where you will meet with a trained counselor and review the results.

Results of this screening are not released or shared with anyone other than the person completing the screening process. If counseling services are recommended, the counselor will explore resource options with you.

Please click below to begin the screening. You will first be required to enter information about yourself. Please enter the information accurately. The brief screening assessment will take 5-10 minutes to complete.

If you are experiencing a mental health emergency and would like to speak with a counselor immediately, please call 800.438.1772 and select option 1. If you are experiencing a life-threatening emergency, please call 911.

These problems are considered significant when you have them for two or more weeks, when they keep coming back, when they keep you from meeting your responsibilities, or when they make you feel like you can't go on.

After each of the following questions, please tell us the last time, if ever, you had the problem by answering whether it was in the past month, 2-3 months ago, 4 to 12 months ago, 1 or more year(s) ago, or never.
